This worksheet becomes the foundation for David to prepare your Last Will and Testament, a General Durable Financial Power of Attorney, an Advance Healthcare Directive (or as some people know them, a Healthcare Power of Attorney and a Living Will – these terms are now outdated due to changes in Georgia law) and the proper HIPAA releases.

Once completed, send the Questionnaire to Mr. Lawler, who will review it with you and advise you accordingly. He will then be able to quote you an exact fee for your custom made Estate Plan. Mr. Lawler will then draft your documents for your review. Once you approve a final version of the documents, an appointment is set for you to come in and formally execute the Estate Planning documents.
